> complete a form to sign up for a name tag
> ask for a key fob
> submit a request for expense reimbursement

Name tags help other know you. Most people like to hear their name when talking to others. It builds fellowship.

The price of a name tag is $14 which is payable: by

cash or cheque at the church office
e-Transfer to office@foresthillunited.org

Members of the church sometimes need a room for a face to face meeting or event. This form will collect the information needed for a room booking. Remember to book early as the rooms have a high level of use.
